Ibruxen 140 mg (Ibrutinib)

Ibrutinib has regulatory approvals in several B-cell malignancies and in chronic graft-versus-host disease, but approved uses differ by jurisdiction.

Chronic lymphocytic leukemia and small lymphocytic lymphoma

Current U.S. IMBRUVICA labeling includes treatment of adult patients with CLL/SLL, including patients with 17p deletion.

CLL is a B-cell leukemia characterized by accumulation of abnormal lymphocytes. SLL is closely related biologically but presents primarily in lymphoid tissues.

Waldenström’s macroglobulinemia

Ibrutinib is also indicated in the United States for adult patients with Waldenström’s macroglobulinemia (WM).

WM is a B-cell malignancy associated with production of abnormal immunoglobulin M.

Chronic graft-versus-host disease

Current U.S. labeling includes adults and children aged 1 year and older with chronic graft-versus-host disease after failure of one or more lines of systemic therapy.

This indication is distinct from treatment of a B-cell cancer.

Mantle cell lymphoma

Regulatory history must be handled carefully. The FDA’s current U.S. labeling no longer lists MCL as an indication. Janssen announced in 2023 its intent to voluntarily withdraw the U.S. accelerated MCL indication.

The European authorization remains different: EMA information currently includes specified MCL treatment settings.

Therefore, an Ibruxen page should not state that every ibrutinib product is approved for MCL worldwide.

Marginal zone lymphoma

The former U.S. accelerated MZL indication was also voluntarily withdrawn. It should not be presented as a current U.S. approved indication.

Approved versus investigational use

Clinical trials may evaluate ibrutinib in additional diseases or combinations. Research activity does not automatically establish regulatory approval.

Treatment combinations

Depending on jurisdiction and indication, ibrutinib may be used alone or with other anticancer medicines. Current U.S. labeling describes combinations with rituximab, obinutuzumab and bendamustine/rituximab for CLL/SLL, and rituximab for WM.
